I slutningen af anden sæson, blev Melinda (Jennifer Love Hewitt) konfronteret med sin fars spøgelse, som afslørede at hun har en bror. Melindas jagt på sandheden om sin familie bliver endnu mere kompliceret og farlig da hun opdager at hendes egne rødder har forbindelse med byen Grandview som de bor i. Melinda og hendes mand Jim (David Conrad) begynder at undre sig over om det rent faktisk er byen som har "valgt" dem og ikke omvendt. Med hjælp fra en ekspert i det okkulte, professor Payne (Jay Mohr) og en loyal men skeptisk ven, Delia (Camryn Manheim), søger Melinda efter svarene. The dead seek out Melinda's ability to help them relay significant messages and information to the living. Despite her fear, compassion compels her to help these earthbound spirits cross over by completing their unfinished business with the living.
